SafetyNet

Offering Support in Difficult Times

Sudden illness, or the worsening of a prior condition, can throw a family into chaos, especially when their financial situation is precarious. The goal of SafetyNet is to help older adults to continue living in their homes when illness strikes or worsens and there is limited or no availability of caregivers.

Our care managers can do the following for you and/or the person you're caring for:

  • Conduct at-home assessment of person's available supports (e.g. family, neighbors, friends), ability to understand and follow medical instructions, stability of housing, safety issues, self-care abilities, and anticipated needs
  • Facilitate family discussion about upcoming needs and family strengths
  • Review eligibility for government programs (e.g. Medicaid, Social Security, Veterans’ benefits, food stamps, nursing home diversion waiver)
  • Assist individual in applying for government programs
  • Connect individual to community resources
  • Advocate for individual to acquire services
  • Follow up with individual/caregiver to ensure that the arranged services are sufficient for safety and continued at-home living
